Transitioning from Pediatric to Adult Cardiology

The Stanford Center for Inherited Cardiovascular Disease (SCICD) is committed to caring for families with genetic cardiomyopathies and channelopathies across the lifespan.  The pediatric team at Lucile Packard Children's Hospital (LPCH) cares for children and adolescents until eighteen years of age.  A staged transition process in early adolescence is implemented in preparation for adult-oriented care.

Our general approach towards transition to adult cardiology care includes:

  1. A multidisciplinary team involved in a coordinated care model.
  1. Joint meetings twice a month to discuss families and/or transitioned adolescents
  1. Introduction to an adult team member at the last LPCH clinic visit.
  1. Ongoing education and guidance on self-care (i.e. knowing medications, making appointments, calling in prescription refills) 
  1. Guidance on the shifting parental role from caregiver to supporter.
